Blue Phone Request Form-As a service to Yale community members, we are offering the opportunity to request a blue phone online.  Please fill out the form below and your request will be considered.  You may want to visit www.yale.edu/securityawareness to review current blue phone locations so that we may avoid duplication. This form will be sent electronically to the Director of Security Programs for assignment to a Yale Security Project Manager.
